As young people progress through life they, and their families, experience a number of key transition stages on the journey from birth to young adulthood.  As children experience their early years, transition through their school years, and then through to their post school destination physical situations; health, education and social care requirements and emotional dependencies can all change.

These periods can be times of great uncertainty with the success of transitions crucial to further development as well as physical and emotional wellbeing, but it is during these transitional stages that some of societies most vulnerable can be left behind. For those living with long term conditions the management of multiple care plans; the constant adaption to changing situations and the maintenance of numerous relationships can present some real challenges.

This one day conference will bring together all those involved with developing, progressing and maintaining transition pathways for Scotland’s children and young people at some of the most crucial life stages, identifying where there is a need for development and how to better facilitate joined up thinking and service delivery at local, regional and national levels.

Key topics for discussion during the day will include:
  • Identifying need – recognising gaps in service provisions and support, and identifying those most at risk of unsuccessful transitions;
  • Design and delivery – examining and improving relationships between services and service professionals;
  • Addressing inconsistencies – tackling inconsistencies in funding, service qualification criteria, communication and care leads;
  • Joined up thinking – facilitating more partnership and joined up working to improve processes, services and support
